Jennifer Abrams, MD

Founding Partner

Dr. Jennifer Abrams is a Bay Area native who practices deeply personalized, relationship-based concierge medicine at Woodside Health Partners. She believes that the foundation of excellent care is truly knowing her patients - understanding their lives, priorities, and long-term health goals - and partnering with them over time to support lasting wellbeing.

Concierge medicine allows her the time and continuity to build those relationships and to care for patients thoughtfully and proactively. Dr. Abrams’ approach centers on prevention, careful listening, and a long-term perspective on health. She works closely with patients to understand the full context of their lives and health histories, helping them navigate both everyday concerns and more complex medical decisions with clarity and confidence. Her goal is to create a supportive environment where patients feel known, heard, and empowered in their care.

She completed her undergraduate education, medical degree, and Internal Medicine residency at Stanford University. During her undergraduate years she received the Dean’s Award for Public Service, and before entering medical school she was awarded a Fulbright scholarship to develop a public health internship program in Roatán, Honduras. During residency she was recognized with the Julian Wolfsohn Award for her clinical judgment, leadership, teaching, and kindness.

 

In addition to her work caring for patients, Dr. Abrams has served as Physician Director for the Stanford-affiliated mentorship initiative “Be A Good Doctor,” which focuses on cultivating compassion and humanism in the next generation of physicians. She is also an active member of the American Medical Association and the American College of Physicians.

 

Dr. Abrams has practiced concierge primary care since 2014 and continues to practice with the belief that the best medical care grows from strong relationships, thoughtful attention, and a shared commitment to long-term health.
